The NUMRICH 12-gauge-into-.22LR Adapter by DINA Arms does not hold its .22LR round tightly--it will fall out if you try to carry it upside-down in a Shotshell Carrier Card (SSC). You'll have to load a .22LR round into the Adapter pointing down/sideways so God's invisible hand of gravity helps you, and then into your shotgun's breech action.

Elastic Loop SSCs

Elastic Loop 3-Round SSC with 2x Crazy Glue Capsules with 12x .22LR Rounds & the 20mm/12-gauge Adapter on SOFT Velcro attached to the Multi-Cam Trouser 30-round AR15/M16/M4 PRELoaded Magazine (PRELM) Lower Trouser Pocket 

To get .22LR rounds securely onto SSCs, get 2x Crazy Glue capsules and wrap HARD Velcro around their center to fatten them up to fit into the elastic loop, You can now temporarily attach a .22LR capsule to your shoulder's SOFT Velcro if you like. A 3-Round SSC (MEMORY device: THREE is deadLEE) can hold the Adapter and 2x Crazy Glue capsules, each holding 6x .22LR rounds for a total of 12. 

Load 6x .22LR Rounds into the Crazy Glue Capsule, Nose in 1st

You can colored tape or paint the Crazy Glue capsules to differentiate between sub-sonic ammo for max stealth and regular .22LR ammo.  

DIY Velcro-Attach SSCs

Another option is to create your own SSC using Velcro instead of sewn elastic loops. Cut a rectangle of cardboard and peel/stick/attach SOFT Velcro on one side; HARD Velcro on  the other. The Crazy Glue capsules attach well enough but even a HARD Velcro piece pressing down around thecylindrical 20mm/12-gauge Adapter into the SOFT Vecro base was not tight enough. I staple-gunned 6x staples to wrap the HARD Velcro around the Adapter and very tightly down onto the base. I then bent the staples' prongs down with pliers then hammered them flush. 

SLAM! the Hammer Down on these Staple Prongs to Flatten them!

VOILA!
